Legal Department Request Form

 If the agreement/contract amount is greater than $25,000 – Public Bids or Sole Source Justification must be attached and Immediate Supervisor and Administrator must have approved the agreement/contract. 

If agreement is between $5,000-24,999.99 – Supporting documents that three quotes were obtained and must be attached and Immediate Supervisor must have approved the agreement. 

 Must submit policies and procedures with a memorandum of immediate supervisor and administrator approval. 

 Must submit description and purpose of the resolution needed and any supporting documents including any templates if available.
