Job Description 

Position:                  Wraparound Case Worker

Supervisor:             Behavioral Health Director

Division:                  Health and Human Services Administration

Department:            Behavioral Health Program

Responsibilities:

The Wraparound Case Worker provides direct support services to patients enrolled in the Behavioral Health and New Hope Department. This role is highly responsive and includes routine duties during standard hours as well as scheduled on call coverage to respond to urgent needs after hours. The Wraparound Case Worker assists with transportation, coordination of care, basic patient support, and immediate response to concerns related to patient safety, behavior, or needs. 

Essential Duties and responsibilities include the following:

  • Provide safe and reliable transportation for clients to appointments, treatment services, and community resources.
  • Respond to after-hours calls from management regarding patient needs, program issues, or incidents requiring immediate follow up.
  • Maintain on call availability as assigned, including evenings, weekends, and holidays, and respond promptly to all requests during those hours.
  • Provide timely support during patient crises or urgent situations, in accordance with protocols.
  • Submit a detailed incident report following any incident involving a patient, regardless of time of occurrence.
  • Assist patients in accessing reservation services, following up on referrals, and staying engaged in their care plans.
  • Maintain regular communication with patients and staff to support continuity and consistency of services.
  • Document all services, patient interactions, and relevant updates accurately and on time.
  • Participate in staff meetings and coordination calls to ensure consistent communication across departments.
  • Support the Behavioral Health and New Hope Departments with non-clinical patient needs.
  • Perform other work-related duties as assigned by immediate supervisor.

Education/experience requirements:

Minimum Qualifications: Relevant experience in behavioral health, human services, or community support roles. Familiarity with community resources and behavioral health support systems. Experience supporting individuals with behavioral health or substance use needs.

Preferred Qualifications: Associate’s degree in Social Work, Psychology, Human Services, or a related field preferred. Bilingual (English/Spanish) preferred.
