Job Description

POSITION:                          Mid-Level Practitioner (NP or PA)

SUPERVISOR:                    Medical Director/Staff Physician

DIVISION:                           Tribal Academic & Human Services Administration

DEPARTMENT:                  Kickapoo Community Health Center

RESPONSIBILITIES:

Provides general medical diagnosis, care and treatment to patients, under the direction of the Physician. Will provide care to ages across the life span; adults, pediatrics, women’s wellness, and geriatric.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Plans and provides comprehensive Nurse Practitioner/Physician Assistant services to clinic patients according to the Center’s policies and protocols, accepted NP/PA standards and the standards of regulatory agencies.
  • Assesses patient’s health by interviewing patients, obtaining and updating medical histories and reviewing patient records to develop comprehensive medical status reports.
  • Performs physical examinations and preventive health measures within their scope of practice. Techniques include inspection, palpation, auscultation and percussion.
  • Provides women’s health services which includes physical, emotional and social aspects, and disruptions that alter a woman’s quality of life, such as reproductive and hormonal issues, bone health, gastrointestinal stress and urinary incontinence to name a few.
  • Records pertinent data in the EHR including physical findings, plan of treatment and diagnosis, based on patient’s condition.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient’s clinical problems and health care needs. Recognizes abnormal findings and treats or refers to the Physician for further evaluation and/or treatment.
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures, within their scope of practice.
  • Discusses case with Physician and other health professionals to prepare comprehensive patient care plan.
  • Refers patient for appropriate specialty consultation for further care or diagnostic procedures that cannot be adequately provided at the clinic. Provides full and adequate information to the receiving facility.
  • Arranges hospital admissions at the direction of the Supervising Physician; performs hospital rounds, if credentialed, and records appropriate patient progress notes; accurately and appropriately transcribes and executes specific orders at the direction of the Supervising Physician; compiles narrative summaries; completes forms pertinent to patient’s medical records; issues diagnostic orders which must be signed within specified time frame as defined by hospital guidelines.
  • Institutes emergency measures and emergency treatment or appropriate stabilization measures in emergency situations such as cardiac arrest, shock, hemorrhage, etc.
  • Instructs and counsels patients by describing therapeutic regimen, giving normal growth and development information, providing counseling on emotional problems of daily living and formulating wellness and health maintenance plans.
  • Provides community health education and disease prevention programs as needed.
  • Maintains adequate records in a timely manner detailing patient care activities.
  • Assists medical staff in assessing, planning, implementing and evaluating health care for the patients.
  • Participates in the clinic’s Quality Assurance Performance Improvement (QAPI) Program.
  • Maintains professional knowledge and proficiency through continuing education, conferences or workshops.
  • Maintains standard and ethics at all times to include compliance with applicable privacy regulations.
  • Employee must maintain confidentiality, security and integrity of patient data and information according to privacy regulations, including HIPAA.
  • Employee must assure age and culturally appropriate, holistic patient care based on mission, standards and policies; adheres to standards of relevant regulatory agencies.
  • Employee must consistently be courteous to patients, staff and visitors.
  • Perform other work-related duties as assigned by immediate supervisor.

EDUCATION / EXPERIENCE REQUIREMENTS:

Minimum Qualifications for mid-level provider who is a PA: Graduate of a nationally accredited program for the Physician Assistant. Minimum of three years’ experience in public health, community health or outpatient clinic setting. Current certification from the NCCPA (National Commission on Certification of Physician Assistants). Current licensure in good standing to practice as a PA for the State of Texas. Current CPR certification.

Minimum Qualifications for mid-level provider who is a NP: Graduate of a nationally accredited program for Nurse Practitioners. Master’s Degree in health care related field. Minimum of two years’ experience in public health, community health or outpatient clinic setting. Current certification in good standing from their appropriate specialty board. Current licensure in good standing to practice as an Advanced Practice Nurse for the State of Texas. Current CPR certification.

Preferred Qualifications: Knowledge of Kickapoo culture and/or language preferred but not required.
